Nah, it's always within the citation. Often it only refers to part of the citation and not to it as a whole, so it's conventional usage to have it inside. Bear in mind that there may be different a different US convention, but I'm pretty sure that this is the standard latin usage for all.

Yeah but 50 Pokemon was easy to get on Gold/Silver/Crystal and up. It was hard on Red/Blue/Yellow when 50 Pokemon was like completing 1/3 of the game.

Now 50 ain't worth shit.
