MY mod dousent work

ok I am kind making a mod l need some help when l place the Item all l get is a bird head weird looking thing

l have 2 .uc l am happy to PM them this mod gives you cerrten augs there mash is a vial and when you activate the vial it gives you a cerrten Aug

can any one help plz!
